If you are unable to complete your program within the assigned timeframe, you may request additional time by submitting the Course Extension Request Form below.
This form is intended for students who require extra time due to personal, medical, or work-related circumstances. We encourage all students to communicate with the school as early as possible to avoid interruptions in their training.
Important Information
- Submission of this form does not guarantee approval
- All requests are reviewed and approved at the discretion of DMS Care Training Center
- Additional fees may apply depending on the program and length of extension
- Requests must be submitted before your course access expires
- Expired accounts may require re-enrollment
- Students are responsible for maintaining progress in their course and meeting all deadlines unless an extension is formally approved
