Before a Blood Test?

We organized the fasting requirements for different blood tests in a simple and easy-to-understand way.

When scheduling a blood test, many people are told, “Please come fasting.”

However, not every blood test requires fasting. Preparation instructions may vary depending on the type of test, hospital guidelines, and whether additional tests are ordered together.

  • Do all blood tests require fasting?
  • Can you drink water?
  • Is black coffee okay?
  • Can you take your medications?
  • Do BMP, CMP, CBC, and thyroid tests require fasting?

The short answer is: not every blood test requires fasting.

Why Is Fasting Sometimes Required?

After eating, certain components in the blood can temporarily change. Blood sugar levels may rise, insulin is released, and triglyceride levels can increase after meals.

These are normal body responses, but depending on the purpose of the test, they may affect how the results are interpreted.

Tests related to blood glucose, insulin, and triglycerides often require fasting for accurate results.


Tests That Commonly Require Fasting

The following tests are commonly performed after 8–12 hours of fasting. However, instructions may vary by facility, so always follow the directions provided by your healthcare provider or laboratory.

Glucose-Related Tests

  • Fasting Glucose
  • Fasting Insulin
  • C-peptide
  • OGTT / Oral Glucose Tolerance Test

Lipid and Nutrition-Related Tests

  • Triglycerides
  • Lipid Panel
  • Iron / Serum Iron

Do BMP and CMP Require Fasting?

BMP (Basic Metabolic Panel) and CMP (Comprehensive Metabolic Panel) are very common laboratory panels performed in hospitals and clinics.

These panels often include electrolytes, kidney function tests, and liver function tests. In some cases, they also include a Glucose component, which is why fasting instructions may sometimes be given.

However, not every component within a CMP necessarily requires fasting. Tests such as AST, ALT, Creatinine, and BUN often do not require fasting when ordered individually.

The most accurate approach is to confirm fasting instructions based on your specific test order and your healthcare provider’s directions.

Tests That Often Do Not Require Fasting

Common Tests That Usually Do Not Require Fasting

  • CBC / Complete Blood Count
  • HbA1c
  • TSH / Thyroid Stimulating Hormone
  • T3 and T4 Thyroid Hormone Tests
  • Vitamin D
  • Vitamin B12

Additional Blood Tests

  • PT/INR
  • aPTT
  • Hepatitis Panel
  • HIV Test
  • hCG / Pregnancy Test
  • Blood Type Testing

However, if these tests are ordered together with fasting-related tests, fasting instructions may still be given.

Can You Drink Water?

For most routine blood tests, plain water is generally allowed.

In fact, not drinking enough water may lead to dehydration, making veins more difficult to access during blood collection.

However, when blood work is combined with sedation, surgery preparation, or certain imaging procedures, even water may be restricted under NPO instructions.

What About Coffee, Gum, Candy, or Smoking?

  • Black coffee: Although low in calories, caffeine may still affect certain test results.
  • Coffee with cream or sugar: This may break a fasting state.
  • Gum and candy: These may trigger digestive responses or affect blood sugar.
  • Smoking: Smoking may affect blood glucose, hormones, and vascular responses.
If you are preparing for glucose, insulin, or hormone-related testing, plain water is generally the safest option before testing.

Can You Take Your Medications?

Blood Pressure Medications

Patients are often instructed to continue taking blood pressure medications as prescribed. Do not stop medications without medical guidance.

Diabetes Medications / Insulin

Taking diabetes medications or insulin while fasting may increase the risk of hypoglycemia, so patients should confirm instructions with their healthcare provider.

Thyroid Medications

Instructions for medications such as Levothyroxine may vary depending on the provider or laboratory. Patients should confirm timing before blood collection.

Iron Supplements, Biotin, and Vitamins

Iron supplements and biotin may affect certain laboratory results, so patients should inform healthcare providers about any vitamins or supplements they are taking.

Pre-Test Preparation Checklist

  • Check whether fasting instructions are listed in your appointment information
  • If fasting is required, follow the instructed fasting period, often 8–12 hours
  • During fasting, plain water is usually recommended
  • Avoid coffee, gum, candy, and smoking
  • Inform the healthcare team about medications and supplements
  • Always confirm instructions for diabetes medications or insulin
  • Check whether water restrictions apply before surgery or sedation procedures

What If You Accidentally Ate?

If you accidentally eat before a fasting blood test, you should inform the laboratory staff or healthcare provider before blood collection.

Depending on the test, the laboratory may still proceed, note the information for interpretation, or recommend rescheduling for more accurate results.
